What We Need From You

Please provide the following three items so our team can complete your Practice Assessment. We ask that all materials be uploaded within two weeks of receiving portal access to help secure your place in the processing queue and ensure timely assignment to a Data Processing Specialist.

12-Month Procedure Code Production Report

A 12-month production report from your Practice Management Software, broken down by individual procedure codes for the entire practice—not by individual doctor.

Current UCR / Office Fee Schedule

Your current office fee schedule showing the fees charged for dental procedures.

60 Days of Recent EOBs

Recent EOBs, ERAs or payer portal documents representing approximately 60 days of claims.

Helpful, but not required

If readily available, these documents can help us better understand your current payer relationships.

12-Month Procedure Code Production Report I confirm that I have uploaded the practice's 12-month procedure code production report. Current Office UCR / Fee Schedule I confirm that I have uploaded the practice's current office UCR or fee schedule. Most Recent Two Months of Insurance Documentation I confirm that I have uploaded EOBs, ERAs, EFTs and/or other reimbursement documentation for the most recent two-month period.

Confirm only after all required assessment documents have been uploaded.
